The idea of writing this story came from a scientist who explored the crater of a certain mountain. Verne got the idea that maybe it would be possible to descend even further into the earth. Many of Verne’s â€Å"fantasy† stories have come true in the years following his death in the year 1905. A Journey to the Center of the Earth is farthest from reality. Even with the technology we have today, it would be impossible for one to descend into the earth. I met with Louis Sutter on Sunday, March 31, 2013. Daniel is a twenty two year old Jewish white male. He currently resides at 123 Apple Road, Columbia, SC, 29205. He is currently studying full time at USC-Columbia and unemployed. He is not married, but in a two year relationship with his girlfriend, Taylor. He was referred to me from the counseling center at USC-Columbia. He was sent to me for anxiety and stress management. Source of Data I received documents from USC- Columbia’s counseling Center, and health records from his primary doctor. I had consent from the client as well as his counselor and his primary physician. The client followed counselor’s advice and made an appointment with me voluntarily. Description and development of presenting problem Louis is currently in counseling where his counselor suggested he specifically seek stress and anxiety management. He is a full time student who is struggling to balance the demands of college and time management which has recently triggered a significant amount of stress and anxiety. My client sought my assistance after struggling to complete assignments on their due dates. Family History Louis currently resides in Columbia but is originally from Greenville, South Carolina. In Columbia, he lives with two roommates. Their names are Brandon and Jeffery. They are both Jewish white male students in their twenties. Louis and his roommates take part in a Jewish fraternity. Louis is the second child to Jane and Rueben Sutter. His parents are middle aged. He has an older sister named Jori who is twenty seven. Louis is very close to his  mother, Jane. Louis is also close to his maternal grandfather. He does not interact that much with his paternal family. Client History Louis was born on March 20, 1991. He graduated high school in 2009. He enrolled in college and plans to graduate May 2014. In 2007 he reports that he struggled with the academic demands of high school. In that same year, documents note that Louis was tested for learning disabilities and psychological disorders. His physician and psychiatrist suggested medication and therapy. See attached document for specific past medications. After some trial and error, my client is currently taking Pristiq and Adavan. My client and medical records show that he takes 80mg Pristiq daily and 50mg Adavan when needed. My client says he is satisfied with his meds and the dosages. He also informed me that his mother and sister suffer from anxiety and depression. My client suggests a healthy intimate relationship because he has been dating the same girl for two years and is content. He has no criminal record or legal issues which were verified with a background check. He mentioned that he has never had an issue with drugs or alcohol. Current Situation Despite the anxiety and stress, Louis appears to be a positive, healthy and happy person. He takes part in a Jewish college fraternity, has a good support system, takes part in some Jewish traditions as well as participates in activities related to his major. He seems enthusiastic about classes, just struggles to meet deadlines, procrastinates as well as feels nervous when doing assignments. He wants to complete assignments confidently but says he is always second guesses himself. My client is a full time student who does not work during when classes are in session. He states that his parents financially support him as well as his saving from working in the summers. He says with his savings and parents help, he is able to pay for food, rent and bills. He has his own transportation and is able to get around town, to classes, internship and social events. He says he is more of a cultural Jew than a religious Jew. The Old Testament God created man with a natural propensity to bet, and the Biblical account of the genesis of Western civilization begins with Adam and Eve wagering a stake, their innocence, in the hope of adding knowledge to that stake. Too many, mans fall from grace and subsequent history are proof it was immoral, or a sucker bet. To others, the wager was an act of self-actualization, with knowledge Adam and Eve acquired humanity, and from the human interdependence created by the loss of Paradise society began. Incidence Reflection - Essay Example In this reflection I have ensured that I maintain confidentiality in line with the Health Professional Council code of Conduct that demands the observance of the same, under code number two that states â€Å"You must respect the confidentiality of service users.† It informs me that I must treat information about service users as confidential and that I must not release any personal or confidential information to anyone not entitled to it (HPC, 2008, pp. 8-9). Therefore all names of the people involved in the process are treated as anonymous and I will different letters to refer to each one of them. I decided to choose this area for my reflection basing on a few reasons. First is that as a student ODP doing my clinical placement, it was my first time to come across such a major incident and therefore it made me curious. Secondly is that, I realized that this procedure is applied to all EG operations, making it commonly used. As a student on placement I realize the need to familiarize myself with the incident and procedures of operation as it is a common phenomenon in our daily lives. The last point is that, I engaged myself in massive research on this topic thereby developing skills and knowledge in this particular area. Rapid Sequence Induction Intubation (RSII) Rapid Sequence Induction and Intubation (RSII) is a medical procedure involving the fast induction of general anaesthesia and following intubation of the trachea. It is generally used in an emergency (EG) situation or for patients who have an increased risk of aspirating gastric contents into lungs (EL-Orbany & Connolly, 2010). The main objective of this technique is to minimise the interval time between loss of protective airway reflexes and tracheal intubation with a cuffed endotracheal tube (ETT). If the intubation is not attained within a maximum 2 minutes, the patient could suffer extreme morbidity or even death from hypoxia (lack of oxygen in body tissue). Therefore airway management is the most important skill for an emergency practitioner. Failure to secure an adequate airway can cause disability (EL-Orbany & Connolly, 2010). The decision to intubate the patient is sometimes very difficult to reach. The difficulty emanates because t he situation requires high clinical experience so as to recognise the signs of an imminent respiratory failure. The concept of RSI was gradually evolved after introduction of Suxamethonium chloride/succinylcholine (paralytic drug) in 1951, and the description of cricoid pressure (CP) in 1961 (EL-Orbany & Connolly, 2010). The procedure include; oxygen administration, rapid injection of a predetermined dose of thiopental/barbiturate (group of drugs), immediately followed by succinylcholine, application of CP and tracheal intubation. It seemed from these components that the term; RSI which is used in both anaesthesia literature and emergency medicine are both inadequate. Because, the technique includes both anaesthesia induction and tracheal intubation, therefore the term RSII is more accurate and descriptive of the technique (EL-Orbany & Connolly, 2010, pp. 18-25) Reflective models My essay will employ the Rolfe et al. According to this logic of representation of information, we opened our presentation with statement of the underlying assumptions on which we operated, and with mentioning of the most fundamental statement about the Church, namely that it is a very specific organisation that shares similarities and differences with business organisations. In the main body of the presentation we outlined specific qualities of religious and business organisations in relation to their structural peculiarities and their gender discourse, and according to a number of parameters that enabled us to make meaningful comparisons, e.g. sources of motivation (profits in business and divine fulfilment in religion), basic world views (survival of the fittest in business, and equality of people before God and their gender difference firmly integrated in religion), sources of justification of prevalent beliefs (capitalist discourse in business, and historic grounding in religion), etc. At this point, we can already see in what ways religion and business are similar, on what points they diverge, and what it means for the gender discourse. One of the early important conclusions that can already be made here is that due to some principal elements of faith the Church is limiting itself in its flexibility in achieving gender equality, which helps us under stand one the main reasons of the gender controversy in application to religious organisations. After the revealing comparative section of the presentation, we moved to the formulation of thesises that comprise the essence of the gender and priesthood problem. We outlined a number of general themes that deserve examination in the light of gender and priesthood problem, and which seem to indicate the main lines to approach it.
